從小一直受到塌鼻跟鼻子太大困擾，但是Augmentation Rhinoplasty手術感覺很複雜，各種Augmentation Rhinoplasty名詞像韓式Augmentation Rhinoplasty、一段式Augmentation Rhinoplasty、兩段式Augmentation Rhinoplasty、三段式Augmentation Rhinoplasty手術等等看得我眼花撩亂。
看網路上的評價，三段式Augmentation Rhinoplasty手術似乎是較多人推薦的，但因為當時還是學生，預算有限，所以一直還是選擇用微整Augmentation Rhinoplasty的方式來調整鼻子，在山根打過微晶瓷跟玻尿酸，剛開始覺得微整Augmentation Rhinoplasty價格比Augmentation Rhinoplasty手術費用便宜，也不用恢復期，很符合我的需求。But unexpectedly, after a while, the hyaluronic acid and Radiesse fillers in my nose will be absorbed by my body, so I have to get them injected again in less than a year to maintain the height of my nasal bridge. This adds up to a lot of money in the long run. And after about 3 or 4 injections, I feel like my nasal bridge has become much wider!
天生鼻子不高、鼻樑寬、鼻頭大、還有微整Augmentation Rhinoplasty的殘留物，三段式Augmentation Rhinoplasty是否可以幫得了我呢？懷著不安的心情，在網路上搜尋了幾位評價較好的醫師預約諮詢，真的親身諮詢後，發現之前在網路上爬文很難理解的各種名稱，其實即使手術名稱相同，不同的醫師解說好像也不太一樣。

三段結構式Augmentation Rhinoplasty
Augmentation Rhinoplasty手術的細節繁雜，Augmentation Rhinoplasty行銷的名稱多，一段、兩段式Augmentation Rhinoplasty、三段式Augmentation Rhinoplasty還有韓式Augmentation Rhinoplasty等名稱，不是業內人士實在很難瞭解內容有什麼不同，甚至即使是名稱相同，不同醫師的作法也會有所出入。
例如似乎是因為有了兩段式鼻模的名稱，所以直接單純放置Ｌ型鼻模的方式就順理成章地變成了一段式，但同樣說是兩段式Augmentation Rhinoplasty，有的醫師使用Ｌ型鼻模Augmentation Rhinoplasty、有的用Ｉ型鼻模，有的是取耳軟骨作為鼻頭頂高的做法，有的醫師會再取鼻中隔軟骨來做鼻頭加高以及延長的支撐。鼻子與眉心是一個連續，眉骨的高度若是不足，常常在Augmentation Rhinoplasty後變得更加明顯，因此可以利用各種方式隆高眉心，於是變成了三段式Augmentation Rhinoplasty。
Augmentation Rhinoplasty手術用各種方式撐高鼻子，最大的壓力會在鼻頭，所以不管是幾段式的Augmentation Rhinoplasty，都必須要能克服撐高鼻頭後的組織壓力，鼻頭部分若是單純使用外來物撐高（矽膠、卡麥拉、人工骨…等），由於組織並不會跟外來物融合，組織的壓力會跟外來物作對抗，必須找到壓力的平衡點，不然長期下來不是組織受傷便是鼻模變形。完全利用自體軟骨來做鼻頭的支撐自然是安全得多，缺點是傷口較大，手術較複雜，因此手術時間較長。
下鼻軟骨就像是鼻頭的屋頂樑柱結構一樣，撐著鼻頭的組織，全鼻頭自體軟骨利用鼻中膈軟骨做鼻頭撐高、延長的大樑，頂高了下鼻軟骨，帶著鼻頭組織一起增高。然後再利用耳軟骨做頂樓加蓋，讓鼻子的高度能配合上鼻樑Ｉ字型鼻模的高度。如此一來，鼻頭的組織全部為自體組織，增高鼻頭所造成的組織壓力還是能夠安全的承受，提高手術後組織的穩定性。這樣的做法可說是結構式Augmentation Rhinoplasty，也是比較推薦的做法。
另外荊醫師也讓我看了一些結構式Augmentation Rhinoplasty合併Alar Reduction、敲鼻骨的手術案例，對於我覺得自己鼻子太粗獷的視覺感會改善很多。而且這些手術是可以一站式進行的！以有限的手術時間創造更滿意的術後成果，並且恢復期只要一次！各方面評估之下，一站式整形會是比較吸引我的選擇。
Alar Reduction
鼻翼的高度、弧度、厚度皆會影響鼻型外觀，鼻翼緣大小也決定著鼻孔的大小及美觀程度，常見的情況如：鼻翼外展或鼻底太寬，造成鼻孔形狀過大過圓，鼻翼後縮造成的鼻孔外露，經由調整鼻翼寬度與改善外展之後，整體鼻型會更加協調且精緻，鼻孔也會呈理想的水滴形或橢圓形，因此愈來愈多Augmentation Rhinoplasty手術會合併進行Alar Reduction手術，讓術後滿意度更高。

Nasal bone tapping surgery
一般來說，透過結構式Augmentation Rhinoplasty手術，除了能夠明顯架高鼻樑，視覺上鼻樑看起來也會有縮窄的效果，但若鼻樑真的過寬、或是既寬又高的情況，單獨進行Augmentation Rhinoplasty手術，效果與滿意度就相對有限，經由將鼻骨向內側位移，藉此達到縮窄鼻樑目的的手術方式，術後鼻樑看起來也就會比原先來得窄。

I felt quite swollen when I first woke up on the first day, so I diligently applied ice packs and went to bed early and didn't check Instagram. The swelling went down almost completely in about five days. When I had the stitches removed, the nurse said I was recovering well.
